The appointment scheduler makes interacting with the office easier and lets you request an appointment at your convenience. You are also welcome to call the office to schedule an appointment. If you need to change an appointment, please call our office staff.

NOTE: if you are a new patient, or your insurance has changed, or you are a candidate for the Optomap, please printout the authorization forms, fill them out and bring them to your appointment.
